The HBI Road Map Module is designed to support public health agencies in identifying priorities and taking action towards improving dementia and caregiving as guided by CDC’s Healthy Brain Initiative (HBI) Road Map Series. See here for additional resources and information about the HBI Road Map Series. The required content will provide you with an overview of the HBI Road Map for State and Local partners and an action planning template. Learners will be able to:

·       Build familiarity with the HBI Road Map and the public health actions to address dementia and caregiving.

·       Identify resources to support implementation of the HBI Road Map actions.

·       Leverage tools for moving from planning to implementation of the HBI Road Map actions.

Tools within this section fit into the planning phases, including action plans to state health assessments and state health improvement plans (SHIPs). The primary audience of this section is state health agencies at various stages of using the HBI Road Map Series, such as states with or without BOLD Program funding or states interested in cross-walking SHIP and state plans on aging.

Tools within this section fit into the implementation phases. The primary audience of this section is state health agencies with comprehensive state plans in place for using the HBI Road Map Series, such as states with BOLD Public Health Program funding.

It is intended for use by state and territorial public health and aging professionals. Dementia Caregiving and the Role of Public Health Agencies

Partnerships to consider include medical providers, pharmacists, transportation authorities, financial institutions, and state legislatures. Resources focus on the Road Map topics of caregiving, risk reduction, early detection, data, and educating professionals.

These posters, flyers, provider guide, radio public service announcements, and short videos about the connection between heart health and brain health were co-developed with and for tribal members by ASTHO and the International Association of Indigenous Aging (IAsquared). The page also includes editable template for a general population, designed by the Alzheimer’s Association, ASTHO, and IAsquared. If using the ready-to-use resources for American Indian and Alaska Native communities, we recommend partnering with tribal serving organizations and/or tribal nation(s).
